APA style recommends that the author's note include author's name, author's departmental affiliation, acknowledgements, and a point of contact for the interested reader.  APA style recommends a form, au author's first name, middle initial(s), and last name.  APA style recommend to omit titles (e.g. Dr., Professor, M.D.) and degrees (e.g. PhD, EdD, MA).  

Author's notes present author name, department name, and university name.  If you have more than one author. You can add that information as well. 

For example, 

One author

Author name, Department name, University name.

John Doe, Department of English, Bowling Green State University. 

More than one author

Author name, Department name, University name;

Author name, Department name, University name;

Author name, Department name, University name.
